Utility Disconnect Form
Prior to demolition of any structure, the owner of the property or his representative is required to confirm that all utilities to the property are disconnected in a manner which does not endanger the life or health of the public and is in conformance with the guidelines established by the utility company or provider.
Please complete this form to the Wheaton Building and Code Enforcement Department within a minimum of three days prior to the actual demolition date to confirm that each company was contacted.
