Please submit the following information:

  • Is this permit for a City of Wheaton-owned water tower?
  • If YES - Refer to steps below, along with providing required submittals 1-4.
  • If NO - Refer to required submittals 1-4.

Consent and Notice of Alterations - Switching out equipment

  • Lease Amendment draft will be created by administrative assistant and reviewed by the City Attorney. Must include approved drawings as Exhibit.
  • Lease Amendment will be sent back to cellular provider for signatures. Once cellular provider has signed (digital is ok), they will return it to the administrative assistant to be signed by the City Manager.
  • Lease will be signed by City Manager after cellular provider signs.
  • Permit will not be issued until all parties have signed.
  • Copy of fully executed agreement will be sent to cellular provider and Director of Building & Code for permit issuance by the City of Wheaton Building Department.

Required Submittals for all projects:

  1. Scope of work Document (Reviewed by Building Director).
  2. Architectural Plans/Specifications, signed and sealed by an Illinois licensed professional, detailing the alterations/additions/new installation.
  3. Structural Calculations, signed and sealed by an Illinois-licensed professional.
  4. Type III Grading Plan (if applicable) - if an electrical equipment cabinet is proposed on a foundation.
